Care Coordinator (Full-time) - 1280 Main Street, Buffalo location. Spectrum Human Services is seeking a candidate to coordinate services in the community for individuals with co-occurring issues, including mental health, physical health and substance abuse. Duties include, completing comprehensive health assessments and patient centered plans, consulting with multidisciplinary team and physicians and preparing of client intervention plan. Candidate must also be an advocate for services and assist with scheduling while conducting case conferences with an interdisciplinary team to monitor and evaluate client status.
