Summary of Position Function

This member of the nursing team is a multidisciplinary team member responsible for conducting nursing assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and support services; and sharing management responsibility with Nurse Manager.  Supervisory duties include oversite of clinic nursing staff to ensure that all services delivered are in accordance with agency standards of practice and core values.

Education Requirements

  • Master’s, Bachelor’s, Associate Degree in Nursing

LICENSURE/CERTIFICATE:

  • Must be licenses as a Registered Nurse in the state in which the program operates

Experience

  • Minimum of three (3) years of work experience in an inpatient or outpatient psychiatric setting, which assures sufficient knowledge and experience applicable to this position.

Must possess a valid Driver’s License with a satisfactory driving record, and possess a personal vehicle for job requirement

Major Duties & Responsibilities

  • Knowledge and demonstration of agency core values in day-to-day activities.
  • Ability to work effectively with clients, families, staff, and community contacts from a variety of cultural and ethnic backgrounds.
  • Conduct comprehensive assessment of medical and physical health history.
  • Provide leadership and supervision to clinic nursing staff to include training and development.
  • Provides continual oversight to ensure members of the team are meeting CCBHC/OMH/OASAS requirements.
  • Assess and responds to medical emergencies and recommends direction for next level of care.
  • Reviews and assures logbooks and checklists are completed accurately and within time frame according to established policy.
  • Maintains inventory of pharmaceuticals and medical supplies.
  • Demonstrates the ability to effectively use technology and appropriate use of support.
  • Assure all documentation is completed accurately and within time frame according to established policy.
  • Monitors Clinic Nurse email group to ensure timely response and resolution of medication needs.
  • Reviews medical information and make medical recommendations on information collected upon intake by clinical staff.
  • Reviews medical reports and lab results received and follows up with MD/NPs accordingly.
  • Attends clinic case conference to provide medical recommendations or answer any questions related to health management.
  • Assesses documentation and assessments/reports generated by non-RN staff for medical concerns and responds accordingly.
  • Manages health monitoring referrals in a timely manner and provides appropriate education on individual needs.
  • Assist with clinical responsibilities as necessary.

SKILLS/COMPETENCIES:

  • Demonstrate clear understanding of the characteristics and problems of adults with severe and persistent mental illness.
  • Demonstrate thorough knowledge of effective clinical practices and principles
  • Demonstrate knowledge of psychotropic medications and the management and administration of medication
  • Strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work and have access to helpful adequate, competent, and continuous supports and services.
  • Must possess skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness and respect for client’s rights and personal preference in treatment are essential
  • Exceptional interpersonal skills, judgment, and decision-making skills
  • Must possess excellent oral and written communication skills in English
  • Proficient in the use of computer software such as MSWord, Excel, Outlook, etc.
  • Additional duties as assigned.
