Summary of Position Function

Provides supervisory duties including supervision of a team as well as direct service to assigned caseload. Services include comprehensive assessment, care planning, coordination and integration of services, risk assessment and evaluation, case management and utilization management.

Education Requirements

  • Master’s degree in counseling  from a regionally accredited or New York State (NYS) registered college or university, preferred or
  • Bachelor’s degree in Human Services

Experience

  • At least three years’ work experience in Care Coordination or treatment, previous supervisory experience a plus.

Must possess a valid Driver’s License with an acceptable driving record.

Major Duties & Responsibilities

  • Provides comprehensive case management to assigned caseload.
  • Assure all care planning includes screening-linking-planning and case conferences involving all providers and support network.
  • Provide constant outreach to individuals, family and other involved persons.
  • Assure and maintain linkages to medical, financial, social, rehabilitative, addiction and psychiatric services.
  • Provide effective advocacy both to obtain services required by individuals and to obtain needed systems development or change.
  • Serve as a social change agent by identifying service barriers, participating in the development of solutions, and serving as a systems advocate.
  • Provide required services including crisis intervention, treatment, and support in the absence of viable alternatives.
  • Supervise a team of care coordinators.
  • Provide supervision and peer case review on a regular basis.
  • Work effectively with a Care Coordination team to provide required services to all individuals they work with.
  • Perform recordkeeping, statistical tracking and quality assurance activities in an accurate and timely manner.
  • Serve as the leader of a high performing team.
  • Implement and monitor program policies and work flows.
  • Perform other duties as assigned.

  • Proficient in the use of computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook, etc.
